SUPPORT & SERVICES
Besides our caring staff, other aspects at Country Lane Homes that assist us in providing a warm family-like environment are the many supports and services we offer. Residents of Country Lane Homes are treated as individuals and not as numbers or room occupants. Therefore, we offer several services in our program based on unique needs.
- 24-Hour Staff Supervision
- Case Management
Residents work with case managers in developing a custom goal plan that reflects their individual needs and wants. This goal plan is then shared among all staff members in-order to provide the best care for the individual and to determine what services are required to reach their goals
- Crisis Management
- Medication Monitoring
Staff members closely oversee that necessary medication is taken at the appropriate times in the correct dosage.
- Healthcare Coordination
We assist residents in arranging medical and dental appointments and other appointments as needed. Closely noting times, dates, hospitals, and doctors.
- Education & Training
Since the staff at Country Lane Homes believes every individual has the right and responsibility to lead as normal a life as possible the residents are taught basic skills. Our residents are provided with instruction in money management, medication management and planning, self-care, meal preparation and other household skills/tasks.
- Transportation
We feel it is an integral part of life for individuals to be able to go to different places within their community. We provide transportation to vocational and work programs, places of worship, shopping, financial institutions, day time facilities, medical facilities, and other outside places and events that promote community involvement.
